What a hospital discharge summary should include
A discharge summary is the medico-legal handover between the hospital and the patient (or the next treating doctor), and is routinely asked for by insurers during cashless and reimbursement claims in India. An acceptable summary usually carries these sections:
- Hospital identification — name, address, contact and IPD registration number
- Patient identification — name, age, sex, address and contact number
- Dates of admission, operation (if any) and discharge, with ward or bed
- Chief complaints, brief clinical history and examination findings
- Final diagnosis and any secondary diagnoses
- Investigations done with significant results
- Procedures or surgery performed with date
- Treatment given during hospital stay
- Condition of the patient at the time of discharge
- Discharge medications with dose, frequency and duration
- Diet, activity, follow-up date and emergency warning signs
