    About the Glasgow Coma Scale

    The Glasgow Coma Scale (GCS) is the most widely used bedside score for level of consciousness in head injury, stroke, poisoning and any altered sensorium. It adds three components — eye opening (1–4), verbal response (1–5) and motor response (1–6) — giving a total between 3 and 15. Always record the components separately (for example E3 V4 M5 = 12), because the breakdown carries more clinical information than the total alone.

    GCS Severity Chart

    • → 13–15 — Mild head injury
    • → 9–12 — Moderate head injury
    • → 3–8 — Severe head injury; airway protection usually required

    What is a normal GCS score?

    15/15 — E4 V5 M6 in a fully alert, oriented patient. The minimum possible score is 3, never 0.

    How is GCS scored in an intubated patient?

    Verbal is scored 1 and suffixed with T, e.g. E4 V1T M6 = 11T, since speech cannot be assessed.

    Which component matters most?

    The motor score is the strongest single predictor of outcome, which is why it is always documented separately.
